If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. For your convenience, payments are accepted in cash, personal check, debit cards, Visa, MasterCard, and Discover cards.

Our experienced staff will submit your dental insurance forms on the day of treatment. Most dental claims are submitted electronically in order to decrease the time required to complete a claim. Reimbursement from the insurance carrier is based on the type of coverage provided by the employer and many levels of coverage may be available from the same insurance company. Our staff will do their best to answer questions regarding any insurance issues, but due to the vast number of insurance options and annual changes, it is ultimately the patient’s responsibility to be aware of their coverage.
We make every effort to minimize your out of pocket expense by accepting assignment of benefits from your insurance carrier when available. Assigned benefits may not result in one hundred percent payment of the charges for treatment. Please remember you are fully responsible for all fees charged for dental treatment regardless of your level of insurance coverage.
We provide payment plans through a third party provider, specifically, CareCredit. One year, interest free financing is available through CareCredit as well as longer term, low interest payment options.
